چکیده

Airway complications continue to be an important source of morbidity and mortality following lung transplantation, and bronchial stenosis is the most common manifestation. Such complications have been reported to occur in up to 27% of cases, and 13% of all transplanted patients may require interventional bronchoscopy for treatment. These complications have been attributed to an insufficient blood supply to the donor tracheobronchial tree, promoting ischemia and consequently leading to anastomotic dehiscence, malacia, or stenosis. Significant airway stenosis causes respiratory symptoms, decrease in lung function, and other complications, such as mucus retention and infection. Recently a novel thinwalled silicone self-expandable stent (Polyflex) has been used to treat tracheal stenosis and tracheo-esophageal fistulas. We report a case of treating bronchial stenosis after single lung transplantation using the Polyflex stent.

Acquired Infrarenal Abdominal Aortic Coarctation: Treatment with Percutaneous Self Expandable Stent

Abdominal aortic coarctation is an extremely rare vascular pathology. Its etiology can be congenital or acquired. Here we present a case of acquired infrarenal abdominal coarctation in a 66-year-old woman who complained of low back and legs pain. She had no signs of resting lower limb ischemia, with diminished distal pulses and normal blood pressure in upper and lower extremities. Magnetic reso...

Removal of metallic tracheobronchial stents in lung transplantation with flexible bronchoscopy

BACKGROUND Airway complications are among the most challenging problems after lung transplantation, and Self-Expandable Metallic Stents (SEMS) are used to treat airway complications such as stenosis or malacia at the bronchial anastomosis sites. Several transplantation centers are reluctant to use SEMS since their removal is sometimes needed and usually requires the use of rigid bronchoscopy un...

Balloon-expandable Metallic Stents for Airway Diseases.

Stent placement is an essential treatment for airway diseases. Although self-expandable metallic stents and silicone stents are commonly applied for the treatment of airway diseases, these stents are unsuitable for the treatment of small airway diseases encountered in pediatric patients and lung transplant recipients with airway complications. Currently, only vascular balloon-expandable metalli...

New bifurcated silicone stent for the treatment of posttransplant bronchus intermedius stenosis: new silicone stent in posttransplant stenosis.

In a recent issue of CHEST (August 2013), Oki and Saka 1 published their preliminary results with a new dedicated bifurcated silicone stent (Novatech SA) in the management of malignant stenoses around the primary right carina. We report here our initial experience with this “Oki stent” for the treatment of posttransplant bronchus intermedius stenosis. Given their particular conformation, posttr...

Pii: S1010-7940(99)00091-3

Objective: Bronchial stenosis, malacia and dehiscence are major airway complications of lung transplantation. Our success in managing this problem evolved from the use of semi-rigid dilators, to balloon dilation and placement of a stent, which were initially silicone, thereafter wire balloon-expandable and ®nally wire self-expandable. Methods: From May, 1994 until July 1997, we performed a tota...
